How can anyone *accidentally* swallow headphones?

I can imagine how that would happen. I sleep with earbuds in, and whilst I’ve never had them end up in my mouth in the middle of the night. If you’re sleeping on your side and an earbud is in your ear loosely, it could roll off into your mouth and you could accidentally swallow it if you’re, well, this guy.

Ok, but why would a doctor, that I assume passed the final exams of 4th grade successfully, ask a person to take off all their metals before an x-ray, yet, search for a little metallic object?

Not to mention that it should've appeared super bright in the scan.

Nothing adds up.
